Offers identification number of the trip; date; title of railroad; number of train; termini; kind of fish; date receiver contacted; whether answer receipted or not; name and post office address of applicant; number of cans; kind and number of fish per can; and number of each can. Some forms include water temperature and number of miles of trip, condition upon delivery, and name of water stocked. Remarks occasionally include name of planter or overseer.
